Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to coordinate and assist with accurate CMS MDS Assessments, CCAs, and Care Plans. You will ensure timely submissions and provide education to direct care staff on CMS RAI updates that affect documentation.

The role requires an active RN license, a nursing degree (ADN/BSN), and at least two years of nursing experience, preferably geriatric, with CPR certification by hire.

Qualifications

• Active RN license in good standing.
• Minimum 2 years nursing experience; geriatric nursing preferred.
• CPR certification by hire or during orientation and maintained.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient condition to the Director of Nursing (DON).
• Educate direct care staff on CMS RAI Manual updates impacting documentation.
• Review the Interdisciplinary Comprehensive Care Plan and ensure CMS QIES submissions are ready for billing.
• Participate in PPS-related meetings and provide staff nurse support as needed.
