Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to oversee the timely completion of MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI guidelines. The role supports the interdisciplinary team and ensures accurate documentation and regulatory compliance.

The ideal candidate has an RN license, 2+ years of nursing experience (geriatrics preferred), and CPR certification. The position requires strong clinical knowledge and collaboration with the DON and healthcare staff.

Qualifications

• Associate’s or bachelor’s degree in nursing.
• Active RN license in good standing throughout employment.
• Two years nursing experience; geriatric nursing preferred.
• CPR certification upon hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations.
• Report changes in patient condition identified by MDS Assessment to the DON.
• Provide education to direct care associates on CMS RAI Manual updates.
• Review Interdisciplinary Comprehensive Care Plan and Final Validation Reports for CMS QIES submission.
• Attend PPS meetings and ensure compliance with documentation standards.
• Perform staff nurse duties as needed and maintain professional conduct.

Education

Associate’s or bachelor’s degree in nursing

Active RN license in good standing

Two years nursing experience
