Job summary

Life Care Centers of America is seeking an RN/LPN MDS Coordinator to coordinate and assist with accurate MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual Regulations. The role requires an active RN license, 2 years of nursing experience (geriatrics preferred), and CPR certification at/within orientation.

Responsibilities include education of direct care staff on CMS updates, review of care plans, and ensuring timely submission before billing.

Qualifications

Must have an Associate or Bachelor's degree in nursing.
Active RN license in good standing throughout employment.
Two years of nursing experience; geriatric experience preferred.
CPR certification at hire or during orientation and maintained during employment.

Responsibilities

Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations.
Report changes in a patient’s condition identified by the MDS Assessment to the DON.
Educate direct care staff on CMS RAI Manual updates impacting documentation.
Review Interdisciplinary Comprehensive Care Plans and Final Validation Reports for CMS QIES submission.
