Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to coordinate and assist with accurate CMS RAI-based assessments, care plans, and related documentation. The role requires an active RN license, current CPR, and at least two years of nursing experience with a preference for geriatric care.

The position involves interpreting CMS RAI regulations, coordinating interdisciplinary teams, and ensuring compliance with state and facility policies while maintaining high standards of patient

Qualifications

• Active RN license in good standing
• 2 years’ nursing experience, geriatric preferred
• CPR certification required
• Familiarity with CMS RAI Manual and regulations

Responsibilities

• Coordinate and assist with MDS Assessments, CCAs, and Care Plans per CMS RAI Regulations
• Report changes in patient condition identified by MDS to the DON
• Educate direct care staff on CMS RAI Manual or SNF regulations impacting documentation
• Review Interdisciplinary Comprehensive Care Plan and ensure accuracy
• Review CMS Reports for late submissions and implement preventative processes
• Attend PPS, Triple Check and other meetings as requested
• Perform staff nurse duties when required
• Maintain patient rights, safety, and sanitation standards
• Communicate effectively within an interdisciplinary team
