Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to travel to multiple skilled nursing facilities and complete timely MDS assessments, care plans, and related documentation per CMS RAI Manual regulations. Required: an active RN license, an associate or bachelor’s degree in nursing, CPR certification, and at least two years of nursing experience (geriatric experience preferred).

Responsibilities

Coordinate and submit interdisciplinary MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations.
Report changes in a patient’s condition identified by the MDS assessment to the DON.
Educate direct care staff on CMS RAI Manual or facility regulation updates affecting documentation.
Assist with review of the Interdisciplinary Comprehensive Care Plan.
Review Final Validation Reports and ensure assessments are accepted into CMS QIES before billing.
Attend PPS meetings and other required reviews; perform staff nurse functions as needed.
