Job summary

Life Care Centers of America is seeking an RN MDS Nurse to coordinate and submit interdisciplinary MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI guidelines. The role requires an active RN license in good standing, 2 years of nursing experience (geriatrics preferred), and CPR certification.

The RN MDS Nurse will educate staff on CMS updates, review care plans, and ensure timely CMS QIES reporting.

Qualifications

• RN licensure in good standing in the state
• Two years of nursing experience; geriatric experience preferred
• CPR certification at hire or during orientation

Responsibilities

• Coordinate and submit accurate MDS Assessments, CCAs, and Care Plans per CMS RAI guidelines
• Report changes in patient condition identified by MDS to the DON
• Educate direct care staff on CMS RAI Manual updates affecting documentation
• Assist with interdisciplinary care plan reviews and CMS QIES reporting
• Attend PPS meetings and other required meetings; perform staff nurse duties as needed
