Job summary

Life Care Centers of America is seeking an RN/LPN MDS Coordinator to coordinate and assist with MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual Regulations. You will ensure accurate documentation and compliance across the interdisciplinary team.

The role requires nursing licensure, 2 years of nursing experience (geriatric preferred), and relevant certifications. You will educate staff and participate in essential care planning and quality reviews.

Qualifications

• Associate or bachelor’s degree in nursing from an accredited college or university.
• Currently licensed/registered as RN or LPN with an active license.
• Two years of nursing experience; geriatric experience preferred.
• CRN C certification (clinical compliance).
• CPR certification upon hire or during orientation; CPR must remain current.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary, accurate, and timely MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by the MDS Assessment to the DON.
• Educate direct care staff regarding updates or changes to CMS RAI Manual or Skilled Nursing Facility Regulations that impact documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and attest that all assessments have been completed and accepted into CMS QIES prior to billing; notify Business Office when not ready to bill.
• Review CMS Reports to identify assessments completed or submitted late and develop prevention systems.
• Attend Daily PPS Meeting, Monthly Triple Check, and other meetings upon request.
• Perform functions of a staff nurse as required.
• Exhibit excellent customer service and a positive attitude towards patients.
• Assist in the evacuation of patients.
• Communicate effectively on an interdisciplinary team.
• Read, write, speak, and understand English.
