Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to coordinate and assist with timely MDS Assessments, CCAs, and Care Plans in line with CMS RAI Manual Regulations.

The role requires an RN with an associate or bachelor’s degree, an active license, 2 years of nursing experience (geriatric preferred), and CPR certification. Strong knowledge of regulatory guidelines and teamwork are essential.

Qualifications

• Associate’s or bachelor’s degree in nursing from an accredited college or university.
• Active RN license in good standing in the applicable state.
• Two years’ nursing experience; geriatric nursing experience preferred.
• CPR certification required at hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient’s condition identified by the MDS Assessment to the DON.
• Educate direct care staff about updates to CMS RAI Manual or regulations impacting documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and ensure CMS QIES submissions before billing.
• Identify late assessments via CMS Reports and develop preventive processes.
• Attend Daily PPS Meetings, Monthly Triple Check, and other requested meetings.
• Perform staff nurse functions as required.
• Demonstrate excellent customer service and professionalism towards patients.
• Assist in the evacuation of patients.
• Maintain dependable attendance and good judgment; communicate well on an interdisciplinary team.
