Job summary

Life Care Centers of America is seeking an LPN/RN MDS Coordinator to coordinate and assist with the completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual regulations.

Qualifications include an Associate or Bachelor’s degree in nursing, an active WA RN license, two years of nursing experience (geriatric preferred), CPR certification, and CRN C certification.

Qualifications

• Associate or bachelor’s degree in nursing from an accredited college or university.
• Active RN license in WA in good standing throughout employment.
• Two years’ nursing experience; geriatric experience preferred.
• CRN C Certification (clinical compliance).
• CPR certification at hire or during orientation and kept current.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient’s condition identified by the MDS to the DON.
• Educate direct care staff on CMS RAI Manual updates impacting documentation.
• Review Interdisciplinary Care Plans and Final Validation Reports before billing.
• Identify late assessments and develop processes to prevent recurrence.
• Attend PPS and other required meetings and collaborate as part of the team.
