Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to oversee and facilitate the timely completion of MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI rules and Life Care standards. The role requires a valid RN license, an associate or bachelor’s degree in nursing, and at least two years’ nursing experience, preferably in geriatric care.

Qualifications

• Active RN license in good standing
• Associate’s or Bachelor’s degree in nursing
• CPR certification on hire and maintained during employment
• 2 years of nursing experience; geriatric experience preferred

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations
• Report changes in a patient’s condition identified by the MDS to the DON
• Educate direct care staff on CMS RAI Manual or SNF regulations impacting documentation
• Review Interdisciplinary Comprehensive Care Plans and Final Validation Reports
• Ensure CMS reporting accuracy and notify Business Office when bills are not ready
• Attend PPS and other relevant meetings and participate as needed
• Perform staff nurse duties as required and maintain professional conduct
• Demonstrate excellent customer service and teamwork while ensuring patient safety
