Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to coordinate MDS Assessments, CCAs, and Care Plans in compliance with CMS RAI Manual regulations. The role supports the interdisciplinary team, reviews plans, and ensures accurate documentation for billing and regulatory adherence.

You will communicate changes to the DON and educate staff on CMS updates to maintain quality patient care and regulatory compliance in a post-acute setting.

Qualifications

• Requires nursing degree and active RN license.
• Two years nursing experience; geriatric experience preferred.
• CPR certification upon hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition to the DON.
• Educate direct care staff on CMS RAI Manual updates.
• Review Interdisciplinary Care Plans and CMS reports.
• Ensure timely and accurate documentation for billing.
