Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to lead and coordinate MDS assessments, CCAs, and Care Plans in a post-acute setting. The role requires current RN licensure, strong knowledge of CMS RAI guidelines, and collaboration with an interdisciplinary team to ensure timely, accurate documentation.

The position emphasizes compliance, staff education on CMS updates, and ongoing quality assurance.

Qualifications

• RN licensure in good standing required.
• 2 years nursing experience; geriatric preferred.
• CPR certification required upon hire.
• Nursing degree from an accredited program.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient’s condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI updates impacting documentation.
• Review Interdisciplinary Comprehensive Care Plans.
• Review Final Validation Reports and ensure CMS QIES submissions are ready for billing.
• Identify late CMS assessments and develop processes to prevent recurrence.
• Attend Daily PPS Meeting and Monthly Triple Check.
• Perform staff nurse functions as required.
• Provide excellent customer service to patients.
• Maintain confidentiality and professional conduct.
• Demonstrate dependability and regular attendance.
• Communicate effectively within an interdisciplinary team.
