Life Care Centers of America seeks an RN MDS Coordinator to coordinate and assist with accurate MDS assessments, CCAs, and care plans in line with CMS RAI Manual regulations. The role includes educating staff, reviewing plans, and ensuring timely CMS submissions.

Strong clinical knowledge and RN licensure are essential. Ideal candidates will hold an associate or bachelor’s degree in nursing, RN licensure in good standing, and certifications such as CRN C and CPR.

Qualifications

• CRN C Certification (clinical compliance).
• CPR certification on hire or during orientation.

Responsibilities

• Coordinate and assist with completing MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI Manual updates impacting documentation.
• Review Comprehensive Care Plan and CMS reports for timely billing.
• Participate in mandatory meetings and ensure compliant documentation.
