Job summary

Country Village is seeking a Wellness Coordinator to manage resident wellness programs and care coordination on-site. The role involves scheduling caregivers and CMA’s, partnering with the healthcare team to ensure smooth operations, and supporting multiple departments as needed.

The successful candidate will coordinate care plans, conduct wellness assessments, and maintain clear communication with residents’ families.

Qualifications

• Experience in care coordination, wellness, or a similar role in healthcare or senior living.
• Strong organizational and communication skills, with the ability to manage schedules and work with interdisciplinary teams.
• Proficiency in scheduling software and wellness management tools.
• CNA certification and CMA certification.

Responsibilities

• Schedule and coordinate caregivers and CMA’s to provide efficient resident care, ensuring adequate staffing levels for each shift.
• Collaborate with the care team to manage care schedules for residents, ensuring continuity of care and timely services.
• Conduct wellness assessments and develop personalized plans in partnership with nursing and care staff.
• Maintain communication with families and caregivers to inform them about residents’ wellness plans and progress.
