Responsibilities

• Coordinate caregivers and CMAs to ensure adequate staffing per shift.
• Collaborate with the care team to manage resident schedules and ensure continuity of care.
• Conduct wellness assessments and develop personalized care plans.
• Communicate with families about wellness plans and progress.
• Oversee staff schedules and adjust coverage as needed.
• Maintain records of resident wellness assessments, program participation and health trends.
• Prepare reports on wellness outcomes for leadership.
