Job summary

SCAN Health Insurance is seeking a remote Care Management Leader to oversee a multidisciplinary team focused on the care of our most complex members. The role ensures regulatory compliance, coordination of services, and escalation management to support seniors and beneficiaries in navigating the healthcare system.

The position emphasizes leadership, collaboration across departments, and ongoing staff coaching, with opportunities for growth and a strong wellness and PTO program.

Qualifications

• Required: Bachelor's Degree or equivalent experience.
• Preferred: Graduate or Advanced Degree or equivalent experience.
• 2+ years experience working in managed care, healthcare environment, or case management.
• 2+ years working with seniors and remote work strongly preferred.
• Spanish speakers preferred.
• Bilingual ability strongly preferred.
• Strong knowledge of medical terminology.
• Deep understanding of CMS and DHCS regulations, including Special Needs Plan requirements.
• Deep understanding of local community resources for seniors.

Responsibilities

• Oversee day-to-day operations of a multidisciplinary care management team.
• Define daily priorities for telephonic and field staff and ensure regulatory compliance.
• Monitor performance and quality through reports and audits.
• Act as subject matter expert for benefits/programs and troubleshoot complex issues.
• Maintain current policies, procedures, workflows, and training materials.
