Job summary

SCAN Group is seeking a Behavioral Health Care Coordinator to support older adult members in a remote role. You will assess, plan, coordinate, and monitor holistic needs, including behavioral health and psychosocial support, while navigating referrals and resources across the care continuum.

You will collaborate with care teams, support member engagement, and help resolve escalated issues with compassion and efficiency, leveraging motivational interviewing and strong documentation in EMR systems.

Qualifications

• High School Diploma required.
• Some college preferred.
• 5+ years working in managed care, healthcare environment, or behavioral health case management.
• 2+ years working with seniors and remote work strongly preferred.
• Advanced technical skills for the functional area.
• Basic problem-solving skills.
• Good communication and interpersonal skills.
• Bilingual Spanish strongly preferred: conversational, reading, writing.

Responsibilities

• Perform general and behavioral health screenings to determine referrals and provide appropriate referrals.
• Actively participate in special projects and serve as a SCAN liaison with behavioral health providers.
• Be a point of contact with members, providers and medical groups for benefits/programs, and connect to behavioral health services.
• Use motivational interviewing to understand member goals and address issues, de-escalate when needed.
• Coordinate care with internal/external teams to develop action plans and resolve access-to-care issues.
• Document interactions, progress, and outcomes in EMR and tracking systems.
