Job summary

SCAN Health Plan seeks a senior care management leader to oversee a multidisciplinary team serving our most complex members. You will set daily priorities, ensure regulatory compliance, and guide staff through care coordination, escalation handling, and chronic disease programs.

In this remote role working Pacific Time business hours, you’ll coach staff, monitor performance, and act as SME for benefits, policies, and CMS/DHCS regulations, helping staff navigate challenges and deliver

Qualifications

• Bachelor's degree or equivalent experience required.
• Graduate/Advanced degree preferred.
• 2+ years in managed care, healthcare, or case management.
• 2+ years remote work with seniors preferred.
• Spanish speakers preferred; bilingual ability desired.
• Leadership, problem solving, communication and interpersonal skills.
• Strong organizational and critical thinking abilities.
• Ability to collaborate across multidisciplinary teams while maintaining confidentiality.
• Deep knowledge of medical terminology and CMS/DHCS regulations.
• Understanding of local community resources for seniors.

Responsibilities

• Supervise day-to-day operations of a multidisciplinary team to meet goals.
• Monitor regulatory compliance, clinical standards, and internal policies.
• Serve as SME for benefits, programs, and escalated access issues.
• Coach staff and develop leadership through performance plans.
• Maintain current procedures, workflows, and training materials.
• Lead meetings, document accurately, and balance competing priorities.
• Apply motivational interviewing to address member concerns and de-escalate conflicts.
• Collaborate with providers and internal/external departments to deliver care.
