Job summary

SCAN Health Plan seeks a dynamic leader to oversee a multidisciplinary care management team, guiding telephonic and field staff to achieve member care outcomes. The role ensures regulatory compliance, quality, and escalation handling while promoting compassionate, person-centered care for seniors enrolled in Medicare/Medi-Cal.

Qualifications

• Bachelor's degree or equivalent experience required.
• Graduate or advanced degree preferred.
• 2+ years experience in managed care, healthcare environment, or case management.
• 2+ years working with seniors; remote work strongly preferred.
• Spanish speakers preferred.
• Bilingual ability strongly preferred.
• Strong interpersonal, written and verbal communication skills.
• Ability to collaborate within a multidisciplinary team and maintain confidentiality.

Responsibilities

• Oversee day-to-day operations of a multidisciplinary care management team.
• Define daily priorities for telephonic and field staff; ensure regulatory compliance.
• Implement and oversee chronic disease management programs.
• Provide coaching, performance updates, and leadership development for staff.
• Act as SME for benefits/program interpretation and escalation resolution.
