Job summary

SCAN Health Insurance is seeking a care management leader to oversee a multidisciplinary team serving the most complex Medicare/Medi‑Cal members. The role emphasizes regulatory compliance, performance monitoring, and proactive coaching across telephonic and field staff.

You will guide team members, address escalated issues with compassion, and ensure alignment with department goals. This is a mostly remote position with Pacific Time Zone business hours and periodic on-site engagements.

Qualifications

• Bachelor's Degree or equivalent experience.
• Graduate or Advanced Degree preferred.
• 2+ years experience in managed care, healthcare environment, or case management.
• 2+ years working with seniors and remote work strongly preferred.
• Spanish speakers preferred
• Bilingual ability strongly preferred.

Responsibilities

• Oversee day-to-day operations of a multidisciplinary care management team.
• Define daily priorities for telephonic and field staff and ensure regulatory compliance.
• Monitor performance and compliance with regulatory requirements and internal policies.
• Serve as subject matter expert for benefits/programs and complex issues.
• Ensure policies, procedures and training reflect current processes.
