Job summary

SCAN is seeking a Behavioral Health Care Coordinator to support older adult members in a fully remote role. You will assess needs, plan, coordinate, and monitor holistic care, including behavioral health and social supports, while guiding members through the health system to improve engagement and outcomes.

Qualifications

• High School Diploma required; some college preferred.
• Experience in managed care, healthcare or behavioral health a plus.
• Ability to work remotely with strong self-management.

Responsibilities

• Screen and refer members to behavioral health services and coordinate care.
• Collaborate with internal/external teams to develop action plans.
• Educate members on care journeys and options to support decisions.
• Document screenings, care plans, and navigation activities in EMR systems.
• De-escalate conflicts and maintain professionalism in sensitive situations.
