Job summary

SCAN Health Insurance is seeking a care management leader to oversee a multidisciplinary team serving complex Medicare and dual-eligible members. The role emphasizes regulatory compliance, quality outcomes, and compassionate service to help members age in place.

You will set daily priorities, supervise field and telephonic staff, and drive chronic disease programs, leveraging motivational interviewing and collaboration across departments to resolve issues efficiently.

Qualifications

• Required: Bachelor's Degree or equivalent experience.
• Preferred: Graduate or Advanced Degree or equivalent experience.
• 2+ years experience in managed care, healthcare environment, or case management.
• 2+ years working with seniors and remote work strongly preferred.
• Spanish speakers preferred
• Bilingual ability strongly preferred.
• Strong interpersonal skills including excellent written and verbal communication.
• Strong organizational skills.
• Strong critical thinking skills.
• Ability to collaborate within a multidisciplinary team and maintain confidentiality.
• Strong knowledge of medical terminology and abbreviations.
• Deep understanding of CMS and DHCS regulations and local senior resources.

Responsibilities

• Oversee day-to-day operations, productivity, and quality of a multidisciplinary team.
• Define daily priorities for telephonic and field staff and ensure regulatory and policy compliance.
• Implement and oversee chronic disease management programs.
• Supervise performance and monitor regulatory compliance and policies; coordinate across departments.
• Provide coaching and education to staff; conduct regular one-on-one meetings.
• Serve as SME for benefits/programs and troubleshoot complex psychosocial and escalated access issues.
• Maintain current documentation and lead special projects, balancing priorities.
• Use motivational interviewing to address member concerns and de-escalate when needed.
