Job summary

SCAN Health Insurance is seeking a remote Care Management Supervisor to oversee a multidisciplinary team serving seniors. The role emphasizes coaching, regulatory compliance, and complex care coordination for dually enrolled beneficiaries across the Pacific Time Zone.

You will lead staff, ensure adherence to CMS and DHCS regulations, coach performance, and advance program goals while maintaining confidentiality and high-quality care.

Qualifications

• Required: Bachelor’s Degree or equivalent experience.
• Preferred: Graduate or Advanced Degree or equivalent experience.
• 2+ years experience working in managed care, healthcare environment, or case management.
• 2+ years working with seniors and working remotely strongly preferred.
• Spanish speakers preferred.
• Leadership - Basic skills to develop others
• Problem Solving - Basic problem-solving skills
• Communication - Good communication and interpersonal skills
• Bilingual ability strongly preferred.

Responsibilities

• Supervise the day-to-day operations of a multidisciplinary care management team to meet department goals.
• Monitor regulatory requirements, clinical standards, and internal policies with regular oversight.
• Act as subject matter expert for benefits/programs and troubleshoot complex issues for Medicare/Medi-Cal beneficiaries.
• Ensure department policies and procedures reflect current processes and compliance.
• Demonstrate strong organizational, analytical, and leadership skills to resolve issues across staff, providers, and departments.
• Facilitate team meetings, document accurately, and lead special projects with multiple priorities.
• Use motivational interviewing to address member concerns and de-escalate when needed.
• Provide staff coaching and education; develop leadership development plans and performance improvement actions.
• Maintain proficiency in essential software and suggest workflow improvements.
