Sentara Health is seeking an Integrated Case Manager (RN) for a remote role with required in-person assessments. Travel to Roanoke and northern areas up to 60-90 minutes per visit, 50-75% of weeks.

Responsible for developing, monitoring, and revising care plans across physical and behavioral health to optimize outcomes. Minimum knowledge includes coordinating with medical directors and interdisciplinary teams, ensuring regulatory compliance, and supporting discharge planning.

Qualificações

• RN licensure required or eligible.
• Discharge planning experience preferred.
• Managed care experience preferred.

Responsabilidades

• Perform telephonic and in-person assessments.
• Develop and monitor the member care plan across physical and behavioral health.
• Coordinate with the care team and providers to implement plans.
• Assist with authorizations/referrals within benefits structures.
