Sentara Health in Roanoke, VA seeks an Integrated Case Manager, RN. This role is remote but requires in-person assessments; travel to Roanoke and northern areas is required up to 60-90 minutes, about 50-75% of the week to see members.

You will perform case management within licensure, develop and monitor care plans, conduct telephonic assessments, coordinate physical and behavioral health, social services and long-term care needs, and ensure compliance with policies.

Qualifications

• Associates degree required, Bachelors preferred.
• Registered Nurse required.
• 3 years nursing experience required; managed care a plus; discharge planning experience preferred.

Responsabilités

• Perform case management services within licensure and develop/monitor/revise the member's care plan.
• Conduct telephonic clinical assessments for identification, evaluation, coordination and management of member needs.
• Identify high-risk members and coordinate care with the health care team; manage chronic illnesses and disabilities.
• Facilitate authorizations/referrals within benefits structure and ensure regulatory compliance.
