Job summary

Sentara Health is seeking an Integrated Case Manager, RN. This role is remote but requires in-person face-to-face assessments with occasional travel to Roanoke and northern areas, up to 60–90 minutes to see members, about half to three-quarters of the week.

You will develop and monitor the member care plan across the continuum, perform telephonic clinical assessments, coordinate authorizations/referrals, and collaborate with Medical Directors and care teams to ensure compliant, cost-effective

Qualifications

• Associates or Bachelors preferred.
• RN license required.
• 3 years nursing experience required; managed care preferred.
• Discharge planning experience preferred.

Responsibilities

• Develop and monitor member care plans across the care continuum.
• Perform telephonic clinical assessments to identify and manage needs.
• Coordinate authorizations/referrals within benefits structure.
• Interface with Medical Directors and care teams on treatment plans.
• Present cases at case conferences for multidisciplinary focus.
