Sentara Health is seeking an Integrated Case Manager, RN. This remote role requires in‑person assessments and travel to Roanoke and nearby areas up to 60–90 minutes, ~50–75% of the week.

You will develop and manage care plans across the continuum, coordinating physical and behavioral health, social services, and long‑term care needs. Responsibilities include telephonic assessments, care coordination, and collaboration with Medical Directors and interdisciplinary teams while ensuring regulatory

Qualifications

• Associates degree required; Bachelors preferred.
• Registered Nurse (RN) required.
• 3 years nursing experience required; managed care and discharge planning preferred.

Responsabilités

• Provide case management across the care continuum.
• Perform telephonic clinical assessments to identify, evaluate, coordinate and manage member needs.
• Identify high‑risk members and coordinate care with the health care team.
• Develop, monitor, and revise member care plans to optimize health outcomes.
• Facilitate authorizations/referrals within benefits structures or extra-contractual arrangements.
• Interface with Medical Directors and care teams; present cases at case conferences.
• Ensure compliance with regulatory, accrediting and company policies and procedures.
