Job summary

Sentara Health is seeking an Integrated Case Manager, RN for a remote role requiring in-person face-to-face assessments. Travel to Roanoke and northern areas is required up to 60-90 minutes, 50-75% of the week.

Responsibilities include developing care plans, coordinating with the care team, and ensuring quality, cost-effective utilization of health benefits. Requires RN licensure and strong communication skills; extensive travel may be necessary.

Qualifications

• Associates degree required.
• Bachelor's degree preferred.
• Registered Nurse is required.
• 3 years of nursing experience required.
• Managed care experience preferred.
• Discharge planning experience preferred.

Responsibilities

• Perform case management services within licensure scope, develop and monitor care plans.
• Conduct telephonic clinical assessments for coordination of physical, behavioral health, social services and long-term care needs.
• Identify high-risk members and coordinate with care team to optimize health outcomes.
• Assist with authorizations/referrals within benefits structure and contracts.
• Present cases at multidisciplinary conferences and ensure regulatory compliance.
