Job summary

Sentara Health is seeking an Integrated Case Manager, RN. This remote position requires in-person face-to-face assessments and travel to Roanoke and northern areas up to 60-90 minutes, roughly 50-75% of the week.

The role focuses on case management across physical and behavioral health, social services and long-term care, aiming to optimize member health and utilization of benefits. Bachelors degree preferred; managed care and discharge planning experience desirable.

Qualifications

• Bachelor's degree preferred.
• Experience in managed care preferred.
• Discharge planning experience preferred.

Responsibilities

• Develops, monitors, and revises the member's care plan to meet needs.
• Performs telephonic clinical assessments for identification, evaluation, coordination and management of member needs.
• Interface with Medical Directors, Physician Advisors and Inter-Disciplinary Teams on care plans.
• Ensure compliance with regulatory, accrediting and company policies and procedures.
