Job summary

Sentara Health is looking to hire an Integrated Case Manager, RN. This role is remote but requires in-person face-to-face assessments. Travel to Roanoke and northern areas is required up to 60–90 minutes to see members about 50 to 75% of a week.

You will manage case plans, coordinate care across physical and behavioral health, social services and long-term care, and ensure cost-effective utilization of benefits. Strong communication and problem-solving are essential.

Qualifications

• RN licensure required
• 3 years nursing experience required
• Managed care and discharge planning experience preferred

Responsibilities

• Provide case management within licensure scope and develop care plans
• Conduct telephonic clinical assessments for needs identification and coordination
• Coordinate care with providers and interdisciplinary teams across the care continuum
• Facilitate authorizations/referrals within benefits structure and contracts
• Ensure compliance with regulatory and company policies
