Job summary

Sentara Health is seeking an Integrated Case Manager (RN) for a remote role that requires in-person face-to-face assessments. The position involves travel to Roanoke and surrounding areas up to 60–90 minutes, about 50–75% of the week, to assess and coordinate member care.

You will develop and monitor care plans, perform telephonic assessments, and coordinate with health teams to optimize outcomes while ensuring compliance with policies and regulations.

Qualifications

• RN licensure required.
• Minimum 3 years nursing experience.
• Managed care and discharge planning experience preferred.

Responsibilities

• Perform telephonic and face-to-face case management, coordinating member care plans.
• Identify high-risk members and coordinate with care team.
• Facilitate authorizations/referrals within benefits structure.
• Collaborate with Medical Directors and care teams on treatment plans.
• Ensure regulatory and policy compliance.
