Sentara Health is hiring an Integrated Case Manager, RN. This remote-friendly role requires in-person assessments and travel to Roanoke and surrounding areas up to 60-90 minutes several days a week.

You will develop and monitor member care plans within licensure scope, perform telephonic clinical assessments, coordinate physical and behavioral health, social services and long-term services, and participate in multidisciplinary conferences. Strong communication and analytical skills are essential.

Qualifications

• Associates degree required; Bachelor's degree preferred.
• Registered Nurse (RN) is required.
• 3 years of nursing experience required; managed care and discharge planning preferred.

Responsabilités

• Develop, monitor, evaluate and revise member care plans within licensure scope.
• Perform telephonic clinical assessments for health, social services and long-term needs.
• Identify high-risk members and coordinate care with the health care team.
• Facilitate authorizations/referrals and participate in case conferences.
• Ensure compliance with regulatory, accrediting and company policies.
