Sentara Health is hiring an Integrated Case Manager, RN. This role is remote but requires in-person face-to-face assessments and travel to Roanoke and surrounding northern areas up to 60-90 minutes weekly for member visits.

The incumbent develops and manages member care plans across physical, behavioral health, social services and long-term services, coordinating with the health care team to optimize care and cost effectiveness.

Qualifications

• Active RN license required
• Minimum 3 years nursing experience
• Discharge planning experience preferred
• Managed care experience preferred
• Travel required up to 60-90 minutes weekly to see members

Responsabilités

• Perform telephonic clinical assessments to identify, evaluate, coordinate and manage member needs across physical and behavioral health, social services and LTSS.
• Develop, monitor, evaluate and revise member care plans to optimize health across the care continuum.
• Coordinate authorizations/referrals within benefits structure or extra-contractual arrangements.
• Interface with Medical Directors, Physician Advisors and care teams on treatment plans; present cases at case conferences.
• Ensure compliance with regulatory, accrediting and company policies; assist in problem solving with providers, claims or service issues.
