Job summary

Sentara Health is hiring an Integrated Case Manager, RN for a remote role that requires in-person assessments in Roanoke and surrounding northern areas. You will develop and monitor comprehensive member care plans across the care continuum while coordinating physical and behavioral health, social services and long-term supports.

Requirements include an active RN license and a minimum of 3 years nursing experience, with managed care and discharge planning experience preferred.

Qualifications

• Active RN license required.
• Associates or Bachelor's degree preferred.
• Minimum 3 years nursing experience; managed care and discharge planning preferred.
• Remote role, but requires in-person face-to-face assessments.
• Willingness to travel up to 60–90 minutes to see members 50–75% of the week.

Responsibilities

• Provide case management within licensure scope; develop and monitor care plans across the care continuum.
• Conduct telephonic clinical assessments; identify, coordinate and manage member needs.
• Interface with Medical Directors and care teams; present at case conferences.
• Ensure compliance with regulatory and company policies; assist with problem solving with providers.
