Job summary

Sentara Health in Roanoke, VA seeks an Integrated Case Manager, RN. This position is remote but requires in-person face-to-face assessments and travel to Roanoke and northern areas up to 60-90 minutes, roughly 50-75% of the week.

The role involves case management across physical and behavioral health, social services and long-term care, coordinating with medical directors and care teams to optimize member health care across the continuum.

Qualifications

• 3 years of nursing experience required; managed care preferred.
• Discharge planning experience preferred.
• RN certification required.

Responsibilities

• Perform telephonic and in-person assessments to identify member needs.
• Develop, monitor and revise care plans to optimize health outcomes.
• Coordinate referrals/authorizations and collaborate with care teams.
