Sentara Health is seeking an Integrated Case Manager, RN. The position is remote but requires in-person face-to-face assessments with up to 60–90 minutes travel to Roanoke and northern areas, about 50–75% of the week.

The role focuses on case management across physical and behavioral health, social services, and long-term services, with responsibilities to optimize care and ensure appropriate authorizations and referrals.

Competenze

• Bachelor's degree preferred, licensure as an RN.
• Experience in managed care is a plus.
• Discharge planning experience preferred.

Mansioni

• Perform telephonic clinical assessments for identification, evaluation and management of needs.
• Develop, monitor, evaluate and revise member care plans across the care continuum.
• Identify high‑risk members and coordinate care with the health care team.
• Manage chronic illnesses, co‑morbidities and utilization of benefits.
