Job summary

Sentara Health is seeking an Integrated Case Manager (RN) to provide case management services across the care continuum. The role is remote but requires in-person assessments and travel to Roanoke, VA, and surrounding areas up to 60–90 minutes from the patient, several days per week.

The position focuses on coordinating care, evaluating and revising care plans, and facilitating authorizations and referrals within benefits.

Qualifications

• Current RN license is required.
• Associates degree in nursing; Bachelor's preferred.
• Minimum 3 years nursing experience; managed care and discharge planning preferred.

Responsibilities

• Provide case management within licensure; develop and revise member care plans.
• Perform telephonic clinical assessments for identified needs across physical, behavioral health, social services and long-term supports.
• Identify high-risk members and coordinate care with the health care team.
• Assist with authorizations/referrals and interface with Medical Directors and Inter-Disciplinary Teams.
• Ensure compliance with regulatory, accrediting and company policies and procedures.
• Travel to Roanoke/Northern VA areas for in-person assessments up to 60–90 minutes, 50–75% of the week.
