Job summary

Sentara Health is seeking an Integrated Case Manager, RN. The role is remote but requires in-person face-to-face assessments.

Travel to Roanoke and northern areas is required up to 60–90 minutes to see members about half to three-quarters of the week. You will manage case plans within licensure scope, coordinate care across physical, behavioral health, social services, and long-term supports, and work with the health care team to optimize member outcomes.

Qualifications

• Associates/Bachelors preferred
• Registered Nurse (RN) required
• 3 years of nursing experience required; managed care preferred; discharge planning experience preferred

Responsibilities

• Perform telephonic and in-person assessments to identify and manage member needs
• Develop, monitor, and revise care plans to optimize health care across the continuum
• Coordinate authorizations/referrals within benefits structure or extra-contractual arrangements
• Interface with Medical Directors and care teams on treatment plans
• Present cases at case conferences for multidisciplinary focus
• Ensure compliance with regulatory, accrediting and company policies
