Job summary

Sentara Health is seeking an Integrated Case Manager, RN. The role is remote but requires in-person face-to-face assessments and travel to Roanoke and surrounding areas up to 60-90 minutes, 50-75% of the week.

The incumbent will manage case plans, perform telephonic assessments, and coordinate care across physical, behavioral health, and social services, ensuring cost-effective utilization of benefits. Requires RN licensure and 3 years of nursing experience.

Responsibilities

• Provide case management services within licensure; develop and revise care plans.
• Perform telephonic clinical assessments to identify and coordinate member needs.
• Identify high-risk members and coordinate care with the care team.
• Assist with authorizations/referrals within benefits structure.
• Interface with Medical Directors and care teams on treatment plans.
