Job summary

Sentara Health is hiring an Integrated Case Manager, RN. This remote role requires travel to Roanoke, VA for in-person assessments and periodic member visits. You will manage care plans, coordinate services, and collaborate with medical directors and the care team to optimize patient outcomes.

Education includes an RN license with a preference for associates or bachelor’s degrees. Travel is up to 60–90 minutes as needed, with compensation in a market range and comprehensive benefits.

Qualifications

• RN licensure is required
• 3 years of nursing experience required
• Managed care and discharge planning experience preferred

Responsibilities

• Develop, monitor, evaluate and revise member care plans to optimize health care across the continuum
• Perform telephonic clinical assessments for needs identification, coordination and management
• Identify high‑risk members and coordinate care with the health care team
• Facilitate authorizations/referrals and align with benefits structures
• Interface with Medical Directors and Inter‑Disciplinary Teams on care plans
• Present cases at case conferences for multidisciplinary review
• Ensure compliance with regulatory and company policies
• Coordinate problem solving with providers and claims staff
