Sentara Health is seeking an Integrated Case Manager, RN, to manage and coordinate care for members. The role is remote but requires in-person assessments and travel to Roanoke and surrounding areas up to 60-90 minutes, 50–75% of the week.

You will develop and monitor individualized care plans, perform telephonic assessments, and collaborate with medical directors and the care team to optimize outcomes while ensuring regulatory and policy compliance.

Competenze

• RN license required.
• 3 years of nursing experience required.
• Discharge planning experience preferred.

Mansioni

• Perform telephonic and in-person case management assessments.
• Develop, monitor and revise member care plans across the care continuum.
• Coordinate with health care team and providers; facilitate referrals and authorizations within benefits.
• Ensure compliance with regulatory and company policies.
