Job summary

Sentara Health is seeking an Integrated Case Manager, RN. This remote role requires in-person face-to-face assessments and travel to Roanoke and northern areas up to 60-90 minutes to see members approximately 50-75% of the week.

The Integrated Case Manager develops and monitors care plans across physical, behavioral health, social services and long-term services, coordinating with the care team to optimize health outcomes.

Qualifications

• Bachelor's degree preferred.
• Managed care experience preferred.
• Discharge planning experience preferred.

Responsibilities

• Develop, monitor, evaluate, and revise the member's care plan to meet needs across physical, behavioral health, social services and long-term services.
• Perform telephonic clinical assessments for identification, evaluation, coordination and management of member needs.
• Identify high-risk members and coordinate care with the health care team.
• Manage chronic illnesses, co-morbidities and disabilities to optimize health care utilization.
• Facilitate authorizations/referrals within benefits structure or extra-contractual arrangements.
• Interface with Medical Directors, Physician Advisors and Inter-Disciplinary Teams on care plans.
• Ensure compliance with regulatory, accrediting and company policies and procedures.
