Job summary

Imagine Pediatrics is a tech-enabled pediatric group reimagining care for children with special health care needs. This remote role involves telephonic enrollment outreach, coordinating with families and providers, and navigating social services to support care access.

The position requires bilingual Spanish, strong communication, and the ability to work independently. Moderate local travel may be required to support field outreach and on-site engagements with partners.

Qualifications

• 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.
• Proven success in a centralized contact center environment.
• Experience navigating social service agencies and connecting families to resources.
• Strong documentation skills; familiarity with a CRM is a plus.
• Excellent collaboration and teamwork in a fast-moving startup setting.
• Stellar listening and communication skills; adapt messaging for diverse audiences.
• Ability to build trust with caregivers and healthcare partners.
• Strong research and data-mining aptitude and problem-solving.
• Ability to work autonomously with minimal supervision.
• Reliable transportation; willingness to spend time in the field when needed.
• Reliable internet access; bilingual Spanish required.

Responsibilities

• Conduct outreach to caregivers of attributed patients to enroll them in care and ensure seamless onboarding.
• Handle inbound/outbound calls from caregivers, health plans, and discharge planners as part of enrollment.
• Engage with DME companies, home health, pharmacies, pediatricians, and other providers to support enrollment.
• Leverage internal and external resources to sustain engagement and coordinate care.
• Collaborate with providers and engagement specialists to support enrollment in various settings.
• Develop knowledge of SSI and identify eligible caregivers for outreach.
• Document interactions and outcomes accurately in the CRM and care records.
