Job summary

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group delivering 24/7 virtual and in-home care for children with special health care needs. This remote role focuses on enrolling children and caregivers, requiring strong outreach, collaboration, and problem-solving in a startup environment.

You will conduct caregiver outreach, coordinate with health professionals, and leverage community resources to support enrollment and care coordination.

Qualifications

• Minimum 2+ years in care coordination, community health, patient engagement, or related fields.
• Proven success in centralized contact center environments.
• Ability to navigate social service agencies and coordinate resources.
• Strong documentation skills; CRM experience is a plus.

Responsibilities

• Conduct research on attributed patients to identify enrollment barriers and outreach opportunities.
• perform outbound calls to caregivers to enroll children in care and ensure smooth onboarding.
• Reach out to healthcare professionals (DME, home health, pharmacies, pediatricians) involved with attributed patients.
• Leverage internal and external partnerships to support ongoing engagement and care coordination.
• Handle inbound calls from caregivers, plan service coordinators, and discharge planners during enrollment."
• Develop knowledge of SSI program and outreach to eligible caregivers.
• Collaborate with Provider Engagement Specialists for on-site outreach when appropriate.
