Job summary

Imagine Pediatrics is seeking a Patient Engagement Associate II to enroll and onboard children and caregivers into our program. This role will work remotely with occasional travel across the United States and will collaborate with an engagement pod to share accountability for enrollment.

You will conduct outreach to caregivers and healthcare professionals, research patient needs, and address barriers to care. A healthcare background and CRM usage are preferred, with bilingual Spanish required.

Qualifications

• 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.
• Experience with centralized contact centers is a plus.
• Experience with navigating social service agencies.

Responsibilities

• Research attributed patients to understand healthcare needs and enrollment barriers.
• Perform telephonic outreach to caregivers to enroll and onboard children.
• Contact other healthcare professionals involved with attributed patients.
• Collaborate with teammates and providers to support ongoing engagement and care coordination.
• Handle inbound calls from caregivers, plan service coordinators, and discharge planners.
• Develop knowledge of SSI program and outreach to eligible caregivers.
