Imagine Pediatrics is a tech-enabled pediatric medical group focused on children with special health care needs. This remote role supports enrollment and onboarding, with travel as needed.

You will perform outreach to caregivers and healthcare professionals, collect care plans, and coordinate with our pods to ensure a seamless onboarding experience for families across the United States.

Qualifications

• 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.
• Proven success in a centralized contact center environment.
• Experience with resourcing within a patient or family facing role and navigating social service agencies.
• Strong documentation skills; experience with a CRM platform is a plus.
• Ability to work collaboratively in a tight-knit team with shared enrollment accountability.
• Excellent listening and communication skills; tailor messaging to diverse audiences.
• Ability to build trust with caregivers and healthcare organizations.
• Strong research and data-mining aptitude; critical thinking and creative problem-solving.
• Ability to work autonomously.
• Access to reliable transportation; willingness to spend time in the field.
• Reliable internet access required.
• Bilingual Spanish required.

Responsibilities

• Conduct research on attributed patients to understand needs and enrollment barriers.
• Perform telephonic outreach to caregivers to enroll in care and onboard.
• Reach out to healthcare professionals involved with attributed patients.
• Leverage relationships to re-engage enrolled patients.
• Handle inbound calls from caregivers, plan service coordinators, and discharge planners.
