Job summary

Imagine Pediatrics is a tech-enabled pediatric medical group seeking a caregiver enrollment specialist. This remote role requires outreach to families to enroll children in care and coordinate onboarding, with field travel as needed.

You will engage with caregivers and health professionals, leveraging CRM tools and community resources to address barriers to care. Fluency in Spanish is required.

Qualifications

• 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.
• Proven success in a centralized contact center environment.
• Experience with resourcing in patient- or family-facing roles and navigating social service agencies.
• Strong documentation skills; CRM familiarity is a plus.
• Bilingual Spanish required; ability to tailor messaging for diverse audiences.
• Ability to work autonomously and collaboratively with a team.

Responsibilities

• Research attributed patients to understand care needs and enrollment barriers.
• Telephonic outreach to caregivers to enroll children in care and onboard smoothly.
• Outreach to healthcare professionals and partners (DME, home health, pharmacies, pediatricians).
• Leverage community resources to support engagement and care coordination.
• Handle inbound calls from caregivers, service coordinators, and discharge planners.
• Develop knowledge of SSI program and outreach to eligible caregivers.
• Collaborate with Provider Engagement Specialists for onsite outreach when needed.
