Job summary

Imagine Pediatrics, a tech-enabled pediatric care group, is seeking a Patient Engagement Associate II. The role focuses on enrolling children and caregivers into our programs through proactive telephonic outreach and coordinated onboarding.

You’ll collaborate with care teams, providers, and health plans to remove barriers to care while expanding engagement with families and communities. The position is remote with local travel as needed, requires 2+ years in healthcare-related engagement, and

Qualifications

• Minimum 2+ years in healthcare-related care coordination, community health, patient engagement, customer service, or sales.
• Experience in a centralized contact center environment is preferred.
• Experience navigating social service agencies and resourcing for patient/family needs.
• Prior documentation with a CRM platform is a plus.
• Ability to collaborate with a tight-knit team and share accountability for enrollment.
• Strong listening and communication skills with the ability to tailor value propositions to diverse audiences.

Responsibilities

• Research attributed patients to understand care needs and enrollment barriers.
• Conduct telephonic outreach to caregivers to enroll their child in care and ensure smooth onboarding.
• Reach out to healthcare professionals (DME, home health, pharmacies, pediatricians) involved with attributed patients.
• Use relationships with internal and external partners to support ongoing engagement and care coordination.
• Handle inbound calls from caregivers, plan service coordinators, and hospital discharge staff during enrollment.
• Learn SSI program details and outreach to eligible caregivers.
• Coordinate embedded outreach at PCP offices or residential facilities as appropriate.
