Job summary

Imagine Pediatrics is seeking a Patient Engagement Associate II to enroll children and caregivers into our pediatric program. You will outreach to caregivers, coordinate with providers, and support enrollment with strong collaboration and problem solving.

Remote role with local travel, requires healthcare experience, robust communication, and Spanish bilingual skills. You’ll work in a team-focused environment with a commitment to serving children with special health needs.

Qualifications

• 2+ years in care coordination, community health, patient engagement, or related field.
• Experience in a centralized contact center environment.
• CRM documentation experience and ability to navigate social services.

Responsibilities

• Research attributed patients to understand healthcare needs and enrollment barriers.
• Conduct telephonic outreach to caregivers to enroll children in care.
• Reach out to healthcare professionals and external providers involved with patients.
• Engage with teams and community resources to support ongoing enrollment and care coordination.
• Handle inbound calls from caregivers, plan coordinators, and discharge planners.
• Learn SSI program details and outreach to eligible caregivers.
• Collaborate with providers for embedded outreach to support enrollment.
