Job summary

Imagine Pediatrics is a tech-enabled pediatric group delivering 24/7 virtual and in-home care for children with special health care needs. The Patient Engagement Associate II collaborates with an engagement pod to enroll and onboard children and caregivers into the program, using outreach and coordination with healthcare partners.

Two or more years in healthcare-related roles, strong communication, and the ability to work autonomously are required.

Qualifications

• 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.
• Proven success in a centralized contact center environment.
• Experience with navigating social service agencies in patient or family facing roles.
• Strong documentation skills; CRM experience is a plus.
• Ability to collaborate in a tight-knit team with shared enrollment accountability.
• Excellent listening and communication; adapt messaging to diverse audiences.
• Ability to build trust with caregivers and healthcare partners.
• Research and data mining aptitude; strong critical thinking and problem solving.
• Ability to work autonomously and travel locally as needed.
• Reliable internet access and bilingual Spanish proficiency.

Responsibilities

• Research attributed patients to understand needs and enrollment barriers.
• Telephonic outreach to caregivers to enroll children and onboard smoothly.
• Outreach to healthcare professionals (DME, home health, pharmacies, pediatricians).
• Leverage internal/external relationships to support ongoing engagement and care coordination.
• Handle inbound calls from caregivers, service coordinators, and discharge planners.
• Develop knowledge of SSI program and outreach to eligible caregivers.
• Collaborate with Provider Engagement Specialists for on-site enrollment support.
